The information in the Benefits Overview table is based on the setup of benefit plans and any manual changes added by your HR administrator. 

1. Filter: By default, this table will show the benefit plans you have enrolled in. The filter includes two options: All and Enrolled. To view all plans with other statuses, click on the dropdown arrow and select All.
2. Benefit plan type: The table categorizes benefit plans by benefit type.
3. Plan name and status: Your plan status will display next to the plan name, as determined by eligibility settings in the plan setup. If you have enrolled in the plan, click on the plan name to view more information. If a benefit requires Evidence of Insurability (EOI), an EOI Pending tag appears next to the plan name so you can quickly identify enrollments that require additional approval.
4. Payment details: A benefit plan shows the amount you (the employee) will pay, which depends on the deduction frequency. Additionally, you may be able to view an amount the company will contribute to the plan. Not all plans will show payment details automatically because some plans have specific payment amounts based on each employee.
5. Coverage: This indicates the type of coverage you have selected for the plan. Not all benefit plans have coverage levels listed since some benefit plans are only available at the employee level.
6. Effective date: This section shows when the benefit plan will go into effect for you and your dependents (if applicable). 

The Dependents table tracks information added about your dependents (e.g., spouse, child, domestic partner, stepchild, or foster child). The table will show you a few important fields for each dependent. 

When you click Add Dependent, the system will give you additional fields, some required, to enter information about your dependent. Click here to learn more about adding a dependent. 

The Benefit History shows a history of all events/changes in an employee's benefit information within BambooHR.

1. Filter: You can filter benefit history by the type of event/change (e.g., eligibility, plan costs, pay schedule, enrollment), plan type, and/or who made the change. After you set a specific filter or multiple filters, click Apply to view the history.
2. Date/Time: When a change to a benefit plan or employee's benefit information occurred (includes manual updates and automatic eligibility events)
3. Event: Details of the change
4. Plan: The benefit plan affected by the change
5. Changed By: The user who made the change
6. Details: To view more details about the benefit event, hover your cursor over the specific event/change and select Details (see example below). This is also where you can view any comments made about the benefit event/change.
   • Comments are not editable, and there is no option to delete a comment, as the table stores a history of benefit changes.
7. View All History: To view a full history of benefit events/changes related to the employee, click View All History. You will then be able to click Hide History if needed.

If your administrator has enabled beneficiary collection, and you have submitted beneficiary information during enrollment, a Beneficiaries for Life Insurance table on your Benefits tab will appear after your life insurance election is approved and the plan goes into effect.

The table lists the people or entities you have chosen to receive your life insurance benefit. For each one, the following information will be displayed under "Primary Beneficiaries" or "Secondary Beneficiaries":

1. Name
2. Relationship to you (like spouse, child, etc.)
3. Type (a person or entity)
4. Percentage of the benefit they will receive

You can update your beneficiary information directly from this table. Dependents you have already added will show up as options, and their details will carry over—no need to re-enter them. To make changes, click Edit Beneficiaries. You can update beneficiaries, adjust percentages (just make sure the total equals 100%), or remove someone if needed.
