Custom Assessment Questions

BambooHR® Performance Management is accessible to customers on the Pro plan. Users can create custom assessment questions for self and manager evaluations, with a maximum of ten questions. The setup process includes adding text questions and customizing ratings. Once a review period starts, changes cannot be made until the next cycle, with notification sent ten days prior for necessary adjustments. Users can export performance reports, including responses to custom questions, in Excel or CSV formats. The system allows for rating customization, which can be added or removed as needed. How do I create custom assessment questions?

Purpose: To help you add custom assessment questions and ratings to self and manager assessments.

Overview

Add custom questions

When you first set up review cycles in Performance Management, you can add up to ten custom questions to the assessments. These questions will be required for the employee to fill out. 

  1. Standard and custom questions in the left column are for self-assessments, and questions in the right column are for the manager.
  2. Click Write A Question.
  3. Type in a question for an employee, a manager, or both. When the employee or manager takes the assessment, they will type a free-text response under the custom question.  
    • If you would like to add the employee's name to a question, enter [Name] as a placeholder, and the system will autofill the placeholder with the employee's name on the assessment. You can also add [Company] to have the company name autofilled. You must use the brackets [ ] in order for the system to fill in the information correctly.
  4. To customize the rating options, click Customize or Remove Ratings to edit or delete.

Once a review period starts, you cannot make any further changes until the next review period. Because of this, all Full Admin users will receive an email notification ten days before the next review period to make any necessary changes. 

When reviewing the Employee Performance report, you can export the report as an Excel or CSV file in order to get a report on the custom assessment questions and responses.

Custom question ratings

Once you click + Add Rating, you will have the option to customize the ratings for your custom question or use our standard list.

Custom question ratings

Each custom question can have a customized rating list. By clicking Customize Ratings, you can edit or remove a rating and add new ratings to the list. For each custom rating, the employee has the option to fill out additional details about their selected rating on their assessment. To remove a custom question rating click the "X" button next to the question.

To remove the ratings entirely, click Remove Ratings. If you decide you would like to re-add custom question ratings simply click + Add Rating and the rating options will reappear.
